1
The Basics of Defibrillation
  • Steps and Procedures
2
Defibrillation
3
Steps
  • Confirm Pulseless Arrest and initiate CPR
    • CPR should be preformed for 2 minutes prior to delivering a shock if the arrest was UNWITNESSED and CPR has NOT been initiated
  • Apply pads (or paddles with conductive media)
  • Charge to 200J* (or 2 J/kg the 4J/kg for pediatrics)
  • Confirm ECG
  • Clear patient
  • Deliver Shock
  • Resume CPR for 2 minutes
  • Revaluate ECG
  • * More on last slide about energy level
4
Apply electrode paddles.
5
Charge paddles.
6
Reconfirm monitor.
7
Verbally and visually clear all.
8
Deliver a shock.
Then Look Away From the Monitor!!!
9
Resume CPR for 2 Minutes
10
Reconfirm monitor.
(Always Check pulse it there has been a rhythm change.)
11
If rhythm is…
  • Not shockable
    • Asystole:  go to Asystole/PEA algorithm
    • Electrical Activity: check for pulse
      • No pulse: go to Asystole/PEA algorithm
      • Pulse present:  begin postresuscitation care

  • Shockable:  repeat defibrillation sequence (starting at charge pads/paddles)

Lifepak 12 Monitor/Defibrillator
Defibrillation Controls
13
Energy levels for Adult Defibrillation
  • Manual biphasic defibrillators: device specific (typically 120 to 200 J)

  • AED:  device specific

  • Manual Monophasic defibrillators: 360 J

  • Unknown: 200 J
