Clinical Competencies Weekly Tracking Form

Note:  You must be using MS Excel to use this form.  It is available to all students in any of the CCC computer labs.

You must complete this form and email it to Mr. Young (wyoung5232@calhoun.edu) weekly tracking your terminal competencies.  This is an Excel file you should save to your computer and then save a copy of each completed weekly form on your personal computer with a file name such as "J. Smith clinicals Week 1".  The information you input into the spreadsheet should include your clinical skills and patient contacts for the given week and sent back to Mr. Young attached to an email or delivered to his office in other formats such as a jump drive.  When sending this file, include your name and week of clinicals in the subject line of the email.  For example, the subject line of the email should read:  "John Smith Clinicals 6/4/2007 to 6/10/2007".  If you do not have Excel on your personal computer, you are welcome to use the computers on the second floor of the Health Science Center.  If this is the case, you should consider bringing a disk to backup your clinical file.
